2. Private Case Management
Our affordable Private Case Management is recommended for complex cases-ones that require an assessment of the physical, cognitive and functional levels of your loved one in addition to a long-term care referral. Our expert Private Case Managers will perform a screening assessment for your loved one and carefully tailor a care plan that will balance quality care requirements with cost and an appropriate use of resources.
A Private Geriatric Case Manager provides you with:
- Personalized professional support for your loved one’s preferences and needs, including physical, financial, environmental, cognitive, and functional levels assessment.
- Quality, continuity and coordination of care.
- Stress reduction caused by miscommunications and paperwork. Your case manager will provide you will all the information you need to make an informed decision. She can also refer you an attorney who specializes in elder law or to a geriatrician.
